What Information I Hold

- Medical history and other health related information including lifestyle
- Treatment details and related notes (made after each consultation)
I will not share your information with anyone else (other than as required for legal process) without explaining why it is necessary and gaining your consent.
How long I retain your information for?
As the law states I will keep your information for the following periods:
- 7 years after the last treatment (re ‘claims occurring’ insurance)
- For a child 16 years or under, the law regarding children records requires them to be kept until the child is 25 years of age, or if 17 when treated, 26 years of age.

I am committed to ensuring that your personal data is secure. In order to prevent unauthorised access or disclosure, I have put in physical and managerial procedures to safeguard and secure the information I collect from you.

- Therapy information or information related to your health

Therapist’s Rights
- If you don’t agree to your therapist keeping personal information about you, or allow them to use the information in a way they need to for treatments, the therapist may decline to treat you.
- Your therapist has to keep records of treatment for the time stated by laws (as stated above). If you ask them to erase any details about you, they might have to keep these details until that period of time has passed.
